Indications/Uses
Relief of symptoms associated w/ seasonal & perennial allergic rhinitis.
Dosage/Direction for Use
Co-Altria tab Adult & childn & adolescent ≥15 yr 1 tab once daily. Patient w/ mild renal impairment (CrCl 50-80 mL/min) 1 tab (10 mg/2.5 mg) once daily. Patient w/ moderate renal impairment (CrCl 30-50 mL/min) 1 tab (10 mg/2.5 mg) every other day. Co-Altria Ped chewable tab Childn 6-14 yr 1 chewable tab once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to montelukast, levocetirizine or other piperazine derivatives. Severe renal (CrCl <10 mL/min). Co-Altria Patients undergoing haemodialysis. Co-Altria Ped Patients w/ rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.
Special Precautions
Montelukast: Systemic eosinophilia, sometimes presenting w/ vasculitis consistent w/ Churg-Strauss syndrome may occur. Avoid concomitant use w/ aspirin or NSAIDs in those w/ aspirin sensitivity. Levocetirizine: Patients w/ predisposing factors of urinary retention (eg, spinal cord lesion, prostatic hyperplasia); epilepsy & those at risk of convulsion. Pruritus may occur when stopped. Concurrent use w/ alcohol. Inhibits response to allergy skin test; a wash-out period of 3 days is required before performing them. Co-Altria Montelukast: Do not use to treat acute asthma attacks. Not to be substituted abruptly for inhaled or oral corticosteroid. Co-Altria Ped Montelukast: Neuropsychiatric events in adults, adolescents, & childn may occur. Levocetirizine: Pregnancy & lactation.
Adverse Reactions
Headache. Levocetirizine: Somnolence, dry mouth, fatigue. Montelukast: Abdominal pain; URTI. Diarrhoea, nausea, vomiting; elevated serum transaminases levels (ALT, AST); rash; pyrexia.
Drug Interactions
Levocetirizine: Decreased clearance w/ theophylline. Increased plasma AUC, t½ & decreased clearance w/ ritonavir. Decreased absorption w/ food. Concurrent use w/ alcohol & other CNS depressants may cause reduction in alertness & impaired performance. Decreased clearance in patients w/ hepatic impairment. Montelukast: Decreased plasma conc w/ phenobarb. Co-administration w/ CYP3A4 inducers in childn eg, phenytoin, phenobarb & rifampicin. Increased exposure w/ gemfibrozil.
